It was Mary who stopped the car at the beginning of the film, but in the later plot it was Alex who stopped the car.

I think that only the beginning and the end of the whole movie happened in the real world, and everything else happened in Mary's mind like "Fatal ID".

The only fact is that Mary did kill Alex's family, and then was counter-killed so she was wounded and was sent to a mental hospital.
