Although the old man has borrowed Shakespeare's words to point out the theme of the film from the very beginning, "life is ultimately meaningless noise and commotion", but I don't think this will stop you from being curious and yearning for the world. There's no guarantee that dreaming really works better than taking medicine, but I know that no matter what stage of life you're in, the cycle of cause and effect, you're going to meet someone, someone you like, who is full of kindness, and who you are obsessed with making a difference. The tall dark stranger of your life track. But more likely, you won't be able to determine your life "for it" -- you wouldn't have the heart to reject the next perfect stranger, would you? So I think living in the moment, no matter how complicated the "choices" you encounter, in addition to distinguishing between their basic good and bad, for most of the rest, you can only pray for more "luck". There is no way, just because of the non-linearity of life, our life it is always so uncertain.
So really, what do you want? When you look forward to your strangers appearing and saving your life in every subway and crowd, you have to know that strangers will eventually be gone. The meaning of a trip is or not, you can only think about it when you calm down yourself.
Maybe the old man just wants to tell you that don't forget to take medicine when you are dreaming.
