The real monster in the title of episode 74 actually refers to Nina. We always thought that the monster refers to John, but the title of episode 74 is a metaphor for the monster. John's doing so much just because he doesn't want the real monster to wake up. On the hospital bed, John and Tianma told the cause, and when Tianma heard it, he understood, which is why Tianma's expression was distorted when he heard it.
In the fairy tale, one of the two monsters goes east and the other goes west. The monster heading east was a little boy, which was John.
"I don't need a name, and I live happily without a name. Because we are monsters without a name." The little boy ate the monster that went west.
The metaphor is that John ate Nina's monster and wiped out all related memories, in order to prevent Nina from recalling it, because Nina "lives happily without a name".
The name he finally got, but there is no one who can call him. John, what a lovely name.
