Who is the real murderer? Different people have different conclusions on this question. As the author of the original work, the screenwriter's idea should have been the most worthy of consideration. However, after the director's shooting and editing, the original concept is likely to have changed. In the eyes of the audience, each audience has their own ideas. In the end, maybe who is the real murderer, this is not the most important. Just enjoy the movie.
However, without exception, I would like to express my personal opinion. Before the end of the movie, all the murderers are female doctors; in the last elevator, the murderer is more likely to be the heroine. The movie is the heroine's mental journey.
